About The Position

Director of Case Management Houston, Texas The Director of Case Management provides strategic leadership and direction for all Case Management activities, ensuring seamless collaboration with a multidisciplinary team including physicians, nurses, and hospital leadership. The Director of Case Management is pivotal in planning, managing, and evaluating Case Management departments, driving the development and implementation of standardized workflows and systems to optimize patient care and achieve organizational goals. The Director also contributes to establishing key performance indicators (KPIs) to measure success in Care Coordination, regulatory compliance, and leadership development.

Requirements

• Registered Nurse required
• Master’s Degree or must be able to obtain one in two years required
• Bachelor Degree in Nursing required in addition to Master’s Degree
• Director of Case Management experience in acute hospital setting

Responsibilities

• Coordinates and supervises the duties of the clinical Case Managers and Social Workers
• Promotes quality patient care outcomes while supporting appropriate Utilization/Resources Management
• Facilitates the multidisciplinary Case Management process by coordinating the development and implementation of tools and systems for effective, efficient Case Management
• Identifies opportunities to continually improve patient care and services
