RN - Coordinator
About The Position
The Outpatient Nurse Coordinator at ELFHCC is responsible for overseeing and coordinating nursing services to ensure high-quality patient care. The Outpatient Nurse Coordinator functions as part of the care management team, performing a variety of clinical and administrative duties related to patient care. Duties may include quality monitoring, patient assessments, direct patient care, and patient/caregiver/staff education. This role exercises independent judgment and engages in continued communication with the patient, family, providers and community resources to assist the patient in attaining a measure of outpatient health.
Requirements
- Bachelor of Science in Nursing (BSN) or equivalent
- Minimum of 3 years of nursing experience, with a focus on coordination or management roles
- Strong knowledge of nursing principles, healthcare regulations, and ELFHCC policies is essential
- Experience with diverse patient populations and understanding of community health issues

- Some roles may require a Master of Science in Nursing (MSN)
