About The Position

The Outpatient Nurse Coordinator at ELFHCC is responsible for overseeing and coordinating nursing services to ensure high-quality patient care. This role functions as part of the care management team, performing a variety of clinical and administrative duties related to patient care. The coordinator exercises independent judgment and engages in continued communication with the patient, family, providers, and community resources to assist the patient in attaining a measure of outpatient health. Qualified bilingual candidates are strongly encouraged to apply.

Requirements

• Bachelor of Science in Nursing (BSN) or equivalent
• Minimum of 3 years of nursing experience, with a focus on coordination or management roles
• Strong knowledge of nursing principles, healthcare regulations, and ELFHCC policies is essential
• Experience with diverse patient populations and understanding of community health issues

Nice To Haves

• Master of Science in Nursing (MSN) for some roles
• Bilingual
