Requirements
- Active Registered Nurse (RN) license
- Minimum 2 years home health RN experience
- Strong knowledge of OASIS documentation, home health compliance, and care planning
- Proficiency with electronic medical records (EMR) systems and Microsoft Office
Nice To Haves
- Experience with Homecare Homebase (HCHB) preferred
Responsibilities
- Review and approve home health plans of care
- Conduct OASIS assessments and OASIS reviews
- Monitor documentation in Homecare Homebase (HCHB)
- Coordinate patient care, referrals, and clinical documentation
- Ensure visits and documentation meet Medicare, state, and federal compliance requirements
- Support quality assurance, performance improvement (QAPI), and home health regulatory standards
