About The Position

Dedicated and compassionate Hospice Clinical Manager with extensive experience in providing quality end-of-life care to patients and their families. Skilled in managing a team of healthcare professionals, coordinating care plans, and ensuring compliance with regulations and policies. Strong communication and organizational skills with a proven ability to create a supportive and comforting environment for patients in their final days. Oversee daily operations of the hospice facility and ensure delivery of high-quality care to patients Supervise a team of nurses, social workers, chaplains, Home Health Aides and other Ancillary staff members Ensure all disciplines develop and implement care plans tailored to the individual needs of each patient Collaborate with physicians, families, and other healthcare providers to ensure comprehensive and high quality care is provided Audit, monitor and evaluate the effectiveness of care plans and guide the staff to write measurable goals Ensure compliance with regulatory requirements and maintain accurate patient records Provide emotional support and guidance to patients and families during the end-of-life process Assures staff competence and performance levels through making field visits, evaluating documentations and providing in-services Participate in interdisciplinary meetings to discuss patient care and make recommendations for improvement Conduct staff training and development to enhance skills and knowledge in hospice care Directs all patient care services for the team, including volunteer and bereavement, and ensures that services are rendered as integrated components of the interdisciplinary plan of care Assures staff competence and performance levels through making field visits, evaluating documentation Participates in all potential alive discharges to ensure effective discharge planning Participate in on-call rotation Active engagement in intake, referrals, hospice information to families Assures that service issues with patients/families or internal customers are resolved promptly and satisfactorily. Active participation in roll out of new projects to team members Assist team in making patient visits as needed Be a mentor and support to all new hires. Follow them on the field and evaluate performance Lead interdisciplinary group team meetings Oversees the team schedule and coordination of services to insure coverage for all patients on the team 24/7 Ensure that payroll is accurate and complete to include meal breaks Participates in QAPI (Quality Assurance Performance Improvement) program and other benchmarking reports

Requirements

  • Bachelor's degree in Nursing
  • Minimum of 3 years of experience in hospice care, with at least 1 year in a supervisory role
  • Strong knowledge of hospice regulations, policies, and procedures
  • Excellent communication, leadership, and interpersonal skills
  • Ability to work effectively in a fast-paced and emotionally charged environment
  • Compassionate and empathetic attitude towards patients and their families
  • Proficient in using electronic medical records (EMR) and other software
  • Must be able to report in ANX Hospice Burlingame office.
