Requirements

  • Active California RN License (Required).
  • Minimum 2 Year Nursing Experience (Home Health experience preferred).
  • The "Helper" DNA: A genuine desire to collaborate across departments and support fellow clinicians.
  • Tech Proficiency: Comfortable with EMR systems and Apple devices.
  • Highly Organized: Ability to multitask and prioritize in a high-volume office setting.
  • Professional Communicator: Excellent phone manner and interpersonal skills.

  • Home Health experience preferred

Responsibilities

  • Clinical Command Center: Provide essential care coordination and logistical support to field staff.
  • Supply Chain Hero: Oversee patient supply orders, tracking, and delivery so clinicians have the right tools at the right time.
  • Outcome Excellence: Monitor patient care and clinical oversight to ensure excellent outcomes for our homebound community.
  • Proactive Outreach: Perform triage and wellness calls, assisting with clinical follow-up as needed.
  • Seamless Communications: Handle incoming clinical faxes and route them appropriately to keep care moving.
  • Strategic Scheduling: Collaborate with the Scheduling team to meet patient care needs efficiently.
