Scope of work Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
Requirements
- Bachelor's Degree or equivalent related field
- 2 years of relevant experience related experience
- LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
- CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration Upon Hire required
- CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants Upon Hire required
- LIC-Physical Therapist - STATE_MI State of Michigan Upon Hire required
- CRT-Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy Upon Hire required
- CRT-Speech Language Pathologist - ASHA American Speech-Language-Hearing Association Upon Hire required
- CRT-Basic Life Support (BLS) - AHA American Heart Association upon hire required
- CRT-Basic Life Support (BLS) - ARC American Red Cross upon hire required
Nice To Haves
- Master's Degree or equivalent related field
- LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan Upon Hire preferred
