Requirements

Maintains the agency's mission, philosophy and core values.
Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
Always maintains patient confidentiality, including all HIPAA regulations.
Licensed or registered in the state in which currently practices
Must pass a criminal background check & MVR check.
Completed health statement acknowledging ability to perform the duties of the position.
Valid state drivers license.
Proof of current automobile insurance.
Current CPR card; TB testing per agency policy

Responsibilities

Provides constant and ongoing care for elderly and terminally ill patients by evaluating patient needs, creating care plans, and providing end-of-life support to patients and their families.
Works in a collaborative, team-driven environment that requires frequent communication with other Care Team members.
RN regularly assesses and reassesses the nursing needs of the Hospice patient.
Registered Nurse Provides dietary counseling.
RN provides Hospice nursing services, treatments, and preventive procedures.
Registered Nurse initiates nursing procedures appropriate for the patients Hospice care and safety.
RN observes signs and symptoms and report to the physician and IDG members any unexpected changes in the patients physical or emotional condition.
Registered Nurse teaches supervises and counsels the Hospice patient and family members about providing care for the patient.
RN supervises and trains other nursing service personnel.
Registered Nurse develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
RN performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctors orders.
Registered Nurse attends team conferences.
RN follows the policies and procedures of The Care Team.
Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
Registered Nurse always communicates to the supervisor if unable to meet a patients need or perform a procedure.
RN participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
Maintain skills and knowledge.
Registered Nurse coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
HOS RN FACILITY POC RN organizes work schedule and utilizes time management to be able to attend all required meetings.
Registered Nurse complies with agency infection control policies and protocols.
RN assist with orientation, teaching and training as requested.
The clinical record shall contain notes for each service provided.
A clinical note will be completed for each visit made to a patients home (including private residence, assisted living facility, group homes or skilled nursing facility) within 48 hours of the visit.
