About The Position

A healthcare organization is seeking an experienced RN Case Manager for a 13-week contract assignment. This role supports care coordination, utilization review, and discharge planning in an acute care setting, ensuring high-quality, compliant, and patient-centered outcomes.

Requirements

• Minimum of 1.5 years of recent acute care experience (within the past 2 years) in a comparable facility
• Active New York State RN License
• Basic Life Support (BLS) – American Heart Association

Nice To Haves

• Experience with EPIC Electronic Medical Records preferred

Responsibilities

• Conduct utilization review to ensure medical necessity and regulatory compliance
• Collaborate with physicians and clinical teams to clarify and improve clinical documentation
• Develop, implement, and monitor individualized care and discharge plans
• Educate patients, families, and staff on care plans and documentation best practices
• Monitor patient progress and identify and resolve barriers to care
• Maintain accurate and timely documentation within the electronic medical record
• Support quality initiatives, coding accuracy, and compliance with regulatory requirements
