About The Position

URGENT HIRE: Full-Time Hospice RN Case Manager (RNCM) Castle Senior Living, Inc. – Hospice Program Castle Hospice, a part of Castle Senior Living is seeking a compassionate, dedicated Registered Nurse Case Manager (RNCM) to join our team. This full-time role provides care to patients in Racine, South Milwaukee, and surrounding communities . ✨ Position Summary As a Hospice RN Case Manager, you will provide skilled, patient-centered nursing care to individuals and families in their homes. You will: ✔ Perform comprehensive physical, emotional, social, and spiritual assessments ✔ Develop and implement individualized care plans ✔ Deliver skilled nursing interventions aligned with hospice standards ✔ Collaborate with the Hospice Director of Nursing to support 24/7 care coverage ✔ Participate in an on-call rotation (with reduced on-call as staffing grows) Why Join Castle Hospice? Requirements

  • Current Registered Nurse (RN) license in Wisconsin
  • 1+ year of experience in medical/surgical, hospice, home health, or skilled nursing (preferred)
  • Excellent communication and problem‑solving abilities
  • Compassionate bedside manner and a heart for hospice care
  • Valid driver’s license + current auto insurance

Nice To Haves

  • 1+ year of experience in medical/surgical, hospice, home health, or skilled nursing (preferred)
