Your Role: Independence with Support As an RN Case Manager, you will: Lead and coordinate care throughout the hospice journey Build meaningful connections with patients and families Collaborate with physicians, NPs, CNAs, social workers, and clinical leaders Provide holistic care—addressing physical, emotional, and spiritual needs Work independently in the field while backed by a highly supportive team
