At Pathways, our RN Case Managers don’t just provide care—they offer comfort, guidance, and presence during some of life’s most important transitions. Your Role: Independence with Support As an RN Case Manager, you will: Lead and coordinate care throughout the hospice journey Build meaningful connections with patients and families Collaborate with physicians, NPs, CNAs, social workers, and clinical leaders Provide holistic care—addressing physical, emotional, and spiritual needs Work independently in the field while backed by a highly supportive team

Requirements

  • Graduate of an accredited school of nursing (BSN preferred)
  • 1–2 years of experience in hospice, palliative care, home health, med-surg, oncology, public health, or acute care
  • Current Colorado RN license and CPR certification
  • Valid Colorado driver’s license, auto insurance, and ability to travel daily
  • Ability to lift up to 35 lbs
