Requirements

Current Registered Nurse (RN) license in the state of practice.
Strong background in patient assessment and case management.

Nice To Haves

Experience in geriatrics, long-term care, home health, or hospice is highly valued.
Knowledge of discharge planning and managed care principles preferred.

Responsibilities

Conducting comprehensive patient assessments and developing individualized, patient-centered care plans.
Collaborating closely with an interdisciplinary care team to ensure seamless communication and coordination.
Managing discharge planning and transitions of care, ensuring patients and families feel supported every step of the way.
Monitoring patient progress and adapting care plans as needs evolve.
Educating and empowering patients and caregivers with clear guidance, compassion, and advocacy.
Maintaining accurate, timely documentation in compliance with healthcare regulations.
Staying current on best practices in care management to continuously improve patient outcomes.
