Requirements

  • Active Massachusetts RN license in good standing
  • Associate’s degree in Nursing required; BSN or equivalent preferred
  • Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
  • Strong interpersonal skills and the ability to thrive in a collaborative team environment
  • Flexibility, self-direction, and sound clinical judgment
  • Valid driver’s license and access to an insured, reliable vehicle

Responsibilities

  • Conduct ongoing, holistic assessments of patients and families
  • Develop and manage comprehensive care plans in coordination with the care team
  • Facilitate interdisciplinary case conferences and care planning discussions
  • Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support
  • Advocate for patients and families, addressing physical, emotional, and psychosocial needs
