Full Time RN Case Manager
About The Position
Our chosen candidate will support our patients and families in the Charlottesville and Augusta County areas. Conduct patient admissions. Conduct routine patient visits. Coordinate care within a hospice interdisciplinary team to assure patient/family needs are met. Communicate with attending physicians, hospice physicians, and other physicians involved in the patient's care. Demonstrates accountability, flexibility, respect, and integrity. Submits all required paperwork and documentation within 48 hours.
Requirements
- Current Registered Nurse license in the state of Virginia.
- Must have a current Texas Driver's license and dependable transportation.
Nice To Haves
- One (1) years experience in a hospice setting heavily preferred (but not required) as a RN Case Manager or similar role.
