Requirements

  • Active RN License.
  • Previous Home Health experience preferred.
  • Current BLS Certification.
  • Valid Driver's License and Vehicle Insurance.
  • Strong time management and prioritization skills.
  • Excellent communication, assessment skills and attention to detail.
  • Visits are primarily within the Salt Lake Valley; however, candidates must be willing to travel to Utah County as needed.

Nice To Haves

  • Previous Home Health experience preferred.

Responsibilities

  • Provide skilled nursing visits including start of cares, infusions, wound care, medication management, diabetic management, recertifications and CNA supervision.
  • Administer medication, treatments, and therapies as prescribed by the physician.
  • Monitor patient's condition and report any changes to the healthcare team.
  • Assist with activities of daily living and provide emotional support to patients and their families.
  • Collaborate with other healthcare professionals to ensure optimal patient care.
  • Maintain accurate and complete medical records using web-based electronic medical records system.
  • This role includes a monthly on-call rotation, which is compensated additionally
