Requirements
- MN RN license in good standing
- 3+ years of clinical RN experience; home health preferred
- Strong documentation, care planning, and assessment skills
- Reliable vehicle and valid driver’s license
Responsibilities
- Lead comprehensive nursing assessments and individualized care plans
- Conduct OASIS visits and coordinate services across care settings
- Collaborate with physicians and providers to maintain clinical continuity
- Deliver education, emotional support, and transition planning for families
- Ensure accurate documentation and regulatory compliance
