About The Position

This role requires a mix of case management, recertifications, s tarts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. At least 1 year of Case Management experience and home health agency (or equivalent) experience are required . Must have experience with OASIS and NON-OASIS assessments. 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus. Experience and willingness to serve clients of all ages (infants, pediatrics, adults, seniors) To support our growth and the increasing volume of client care throughout Indiana, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (PRN / As Needed) to join our agency and serve existing and returning clients in the greater Indianapolis and Metro areas. This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). This is a PRN position with for long-term employment opportunity that can be built to fit your schedule (travel up to 2 hours is required). For those pursuing career growth: a defined path for advancement to Full Time RN Case Manager, Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported. As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes. With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team. You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience.
  • 1+ years of paid home health / in-home care experience is also required. 2+ years of both are preferred.
  • Minimum of one (1) year Experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
  • Minimum of one (1) years of nursing supervisory experience.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.

Nice To Haves

  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Home care experience preferred.

Responsibilities

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
