About The Position

The case manager coordinates with physicians, nurses, social workers and other health team members to expedite medically appropriate, cost-effective care. The case manager applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. Facilitates resource management activities for inpatients based on identified patients. Coordinates patient’s stay from admission to discharge through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction. Provides psychosocial assessment, resources and intervention to hospitalized patients.

Requirements

  • Graduate of accredited School of Nursing
  • Current Alabama or Multistate Nursing License
  • BLS for Healthcare Providers required by the end of orientation period.
  • ACLS required within 1 year of hire.
  • Three years clinical experience strongly preferred.
  • Comparable combination of education and experience will be considered.
  • Effective communication skills, verbal and written, as well as excellent organization skills are required.

Nice To Haves

  • ACM or CCM preferred.
  • Previous utilization management, discharge planning or hospital based social service experience is very beneficial.
