About The Position

We’re looking for a compassionate and self-motivated Registered Nurse (RN) Case Manager to provide in-home visits and case management for clients throughout Cuyahoga County. Requirements

• Active RN license in Ohio
• At least 1 year of clinical experience (home health or case management preferred)
• Driver’s license and reliable transportation
• Strong assessment and communication skills
• A compassionate, self-starter attitude

Responsibilities

• Visit clients in their homes to provide assessments and wellness checks
• Create and follow individualized care plans that meet each clients unique needs
• Partner with families, physicians, and other care providers to ensure seamless coordination
• Offer education and encouragement to clients and caregivers
• Keep clear, accurate records that reflect the quality of your care
• Bring warmth, empathy, and professionalism to every visit
