St. Jude Children’s Research Hospital is in search of Home Health Registered Nurse who will provide care in SJ Housing and the Community. The Home Health Registered Nurse administers skilled, quality nursing care to patients on an intermittent basis in their place of residence. This position coordinates and directs the home health patient's care based on individual patient needs. This requires the use of advanced pediatric assessment, teaching and decision-making skills. This is performed in accordance with physician orders and plan of care under the direction of the Administrator or designee. The Home Health RN provides comprehensive assessment, planning, implementation and evaluation for a caseload of home health patients in accordance with the plan of care. Makes the initial evaluation visit and regularly re-evaluates the patient’s nursing needs by assessing physical, functional, psychosocial and cognitive status of the home health patient utilizing interview observations and physical exam techniques. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames for applicable patients. Based on evaluations, determines the amount and type of nursing needed by each individual patient. Establishes individualized, measurable goals in consultation with the patient, family and other health care providers. On-call rotation - Weekend and holiday coverage required.
