Requirements

Active RN license in Ohio
At least 1 year of clinical experience (home health or case management preferred)
Driver’s license and reliable transportation
Strong assessment and communication skills
A compassionate, self-starter attitude

Responsibilities

Visit clients in their homes to provide assessments and wellness checks
Create and follow individualized care plans that meet each clients unique needs
Partner with families, physicians, and other care providers to ensure seamless coordination
Offer education and encouragement to clients and caregivers
Keep clear, accurate records that reflect the quality of your care
Bring warmth, empathy, and professionalism to every visit
