RN Care/Case Manager
About The Position
Community Care is hiring a RN Case Manager to join our Family Care Program team. This is a full-time opportunity working out of our Dane County location. $8,000 Sign On Bonus About Community Care Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community. We coordinate and deliver a full range of supportive services that help more than 13,000 Wisconsin residents live safely, confidently, and with dignity. We are Wisconsin-based with local offices supporting each of the 15 counties we serve Our dedicated team works with members to develop care plans to meet their health and social needs Together with our community partners, we’re committed to providing compassionate, personalized care that empowers members to live a healthy and independent lifestyle.
Requirements
- Education: Associates degree in Nursing Required, Bachelors Degree preferred.
- Licensure: Wisconsin Licensed Registered Nurse
- Licensure: Eligibility for long-term care functional screener certification is required
- Licensure: WI Drivers License
- Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required

- Preferred 2 years of work in community nursing
Responsibilities
- Completes discipline-specific initial and ongoing assessments.
- Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
- Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports.
- Follow up on all services to ensure quality/coordination and effectiveness of services.
