Requirements

Current RN license in Pennsylvania
Strong knowledge of OASIS and CMS Guideline
Reliable transportation and strong time management
Valid driver’s license, auto insurance, and reliable transportation
Current CPR certificate

Nice To Haves

Prior home health experience preferred

Responsibilities

Complete OASIS assessments (Start of Care, Recert, Resumption, Discharge)
Provide skilled nursing services: wound care (including complex wounds), IV therapy, and medication management
Support chronic disease management and patient/family education
Work closely with PT/OT/ST and the full interdisciplinary team
Follow CMS, Medicare, and PA DOH standards with confidence and accuracy
