About The Position

The RN Case Manager: Victim Services (RN) is a key member of the Trauma Recovery Medical Home (TRMH) care team at CHC/SEK. This program provides long-term, trauma-informed care for victims of intimate partner violence (IPV) and sexual assault (SA) across a seven-county rural service area in Southeast Kansas (Service Area). The RN delivers individualized, victim-centered services, coordinating with medical, mental health, addiction treatment services, advocacy, and community partners to address the physical, emotional, and social impacts of violence. The RN serves as the central hub of care coordination—conducting intakes and assessments, building care plans, facilitating follow-up, and ensuring wraparound support for victims and their children. The RN working closely with the mental health provider, and other members of the care team, to develop and co-facilitates survivor-focused life skills workshops and peer support groups, to promote resilience, connection, and healing. This position provides both in-person services at CHC/SEK’s Pittsburg locations and at Safehouse Crisis Center’s emergency shelter, as well as telehealth services for victims throughout the Service Area. In addition, the RN supports the lead forensic nurse in providing forensic nursing services, including answering hotline calls and performing medical forensic exams for victims of IPV and SA, as needed. This position engages in cross-sector collaboration, serving as a liaison to victim advocates, law enforcement, protective services, and other members of multidisciplinary community response teams (CRT) for IPV and SA. This position requires advanced clinical and assessment skills, knowledge of community resources, and the ability to collaborate effectively across healthcare, justice, and social service systems.

Requirements

  • Current Kansas RN license is required. Must obtain License to practice in Oklahoma within one year of hire.
  • Minimum three (3) years of nursing experience, preferably in emergency, women’s health, or similar role where advanced assessment skills are required.
  • At least 1 year of case management, or experience relevant to community health, correctional health, or cross-agency collaboration experience preferred.
  • Must complete forensic nurse training (SANE and IPV) within 6 months of hire.
  • Victim Services and Trauma-Informed Care training must be completed within 3 months of hire and on an ongoing basis for continuous improvement, as dictated by the supervisor.
  • Demonstrated leadership experience.
  • Basic Life Support (BLS) certification through American Heart Association or obtain within 90 days of hire.
  • Must possess a valid driver’s license as travel is required. Must maintain a clean driving record of no more than 2 minor or 1 major driving violation in a 3 year period.
  • Ability to work autonomously, with little to no direct oversight, making evidence based, independent decisions in complex, high-pressure situations.
  • Work well as part of a multidisciplinary, collaborative team, building and sustaining relationships with internal and external partners across healthcare, criminal justice, and community-based systems, representing oneself in a manner that reflects the organization’s mission and values.
  • Expertise in trauma-informed care, crisis intervention, and survivor-centered communication. Skilled in delivering education and training to diverse audiences.
  • Proficient in working with technology (e.g., computers, tablets, cameras, etc.) and software (e.g., Microsoft Office, electronic health records, case documentation systems, etc.).
  • Strong organizational skills, ability to manage multiple priorities, and adaptability to frequent or sudden changes, maintaining flexibility with last-minute schedule changes.
  • Maintain patient confidentiality and uphold ethical standards when dealing with sensitive information in accordance with organization’s policy and procedure and HIPAA requirements.
  • Social and cultural sensitivity appropriate to an ethnically and economically diverse patient-and employee-base.

Responsibilities

  • Provide trauma-informed nursing care to victims of IPV and SA.
  • Case management includes performing comprehensive assessments, developing individualized care plans, and coordinating care and services (e.g., medical and dental care, referrals to specialists, and follow-up services) for victims and their children.
  • Maintain regular follow-up to engage patients in their goals, ensure continuity of care, and adjust plans as needed.
  • Partner with CHC/SEK Community Health Workers to address social needs such as housing, food, and transportation.
  • Contribute to the creation and refinement of TRMH workflows and protocols.
  • Collaborate with?the?TRMH team?to organize and co-facilitate?life-skills and peer-support groups, addressing topics like healthy relationships, reproductive health after trauma, parenting after violence, and rebuilding self-worth.
  • Assist the Forensic & Victim Services team with hotline coverage to provide?timely assistance?to callers, addressing their immediate needs around safety, resources, or other information requests.
  • Support the Victim Services Program Manager by providing backup coverage for forensic exams, as needed.
  • Assist Program Manager in tracking TRMH program metrics, patient outcomes, and service utilization to guide quality improvement.
  • Identify gaps in service delivery and develop strategies to enhance victim support.
  • Represent CHC/SEK and actively participate in Coordinated Community Response Team (e.g., CRT, SART) meetings and other community forums, as directed by Program Manager.
  • Engage and collaborate with community partners to strengthen victim identification, improve system-wide responses to IPV and SA, and promote victim engagement in the justice process.
  • Support data collection and grant reporting requirements for state, federal, and internal use.
  • Ensure adherence to HIPAA, safety protocols, and organizational standards for victim confidentiality.
