We are looking for compassionate Registered Nurse Case Managers who want autonomy, flexibility, and the opportunity to make a real difference in patients’ lives. As a Private Duty RN Case Manager, you will oversee and coordinate care for patients receiving in-home private duty services.

Requirements

  • Current Registered Nurse (RN) license in Indiana
  • Graduate of an accredited RN program
  • Strong communication and patient care coordination skills
  • Ability to work collaboratively within an interdisciplinary team
  • Ability to lift, push, pull, or support up to 50 pounds as needed

Nice To Haves

  • 2+ years clinical nursing experience
  • Home Health or Private Duty nursing experience
  • Case management experience

Responsibilities

  • Assess patient health needs and determine appropriateness for home care admission
  • Complete admission assessments and develop individualized plans of care
  • Communicate with physicians, caregivers, and referral sources to ensure smooth care transitions
  • Coordinate services including equipment, supplies, pharmacy, and additional medical support
  • Supervise Private Duty RNs, LPNs, and Home Health Aides to ensure quality and continuity of care
  • Review clinical documentation for accuracy and compliance with the plan of care
  • Evaluate patient progress and adjust care plans when necessary
  • Provide education and resources to patients and caregivers
