About The Position

This hybrid role requires a mix of case management, recertifications, s tarts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. At least 1 year of Case Management experience and home health agency (or equivalent) experience are required . Must have experience with OASIS and NON-OASIS assessments. 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus. SERVICE AREA: 6-county radius in Central-Southern Ohio Includes Pike, Ross, Highland, Brown, Adams, and Scioto Counties. Typical travel distances require a 30-90 minute daily radius (or less), with occasional travel up to 2 hours to cover all county cases also required. JOB LOCATION OPTIONS: Pike County is central to all counties in this service area, but qualified applicants residing in or adjacent to any of the 6 counties will be considered so long as travel requirements are fulfilled. SCHEDULE TYPE: Flexible and customizable. We'll build a schedule as close to home as possible - that fits your availability and preferences. Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm). EMPLOYMENT TYPE: Full time + Long-term employment No On-Call requirements / No Overnight shifts required. Weekend, Overtime, Respite & PRN services are offered when available but are not required. Hourly or Salary Compensation (paid weekly, on Fridays). To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Nurse Supervisor to join our agency and serve existing and returning clients throughout 6 counties in Central-Southern Ohio. This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). This is a Full-time, long-term employment opportunity that can be built to fit your schedule (Full-time preferred; will consider Part-time if necessary). Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred.
  • Minimum of one (1) years of nursing supervisory experience.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.

Nice To Haves

  • Home care experience preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.

Responsibilities

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
