About The Position

Home Care Network is seeking 2 highly skilled and experienced Program Administrators / RN Case Managers (hybrid role) to join our home health agency and direct the business, care team, and agency growth within our Southeast Ohio operations. Given the vast geography and numerous clients we serve in SE Ohio, we're adding 2 Admin/RNCMs to split the coverage and increase support in this region, which primarily includes: Scioto, Jackson, Athens, Vinton, Ross, Pike, Gallia, Highland, and Adams Counties, plus as-needed support in Lawrence, Meigs, Washington, Hocking, Brown, Fayette, and surrounding counties. JOB TITLE: Program Administrator & RN Case Manager (RN license + Case Management experience required) JOB LOCATION: In the field + Remote or Office | Each Admin/RNCM will manage 10-13 counties across Southeast Ohio (est. 2-hour travel radius). RN Case Manager in-field activities and services typically require weekday travel within a 30-60 minute radius, up to 2 hours occasionally / as needed. When not in the field with our employees, clients, and care partners, this role will have the flexibility to work Remotely or in our Corporate Office in Jackson, OH, located at 731 E Main St, Jackson, OH, 45640. In-person support during State Surveys requires presence at our Jackson, OH office. SCHEDULE: A typical week is Monday - Friday from 8 AM - 5 PM, though willingness to provide occasional on-call and varied / flex hour coverage (late night / weekend emergency) is required as needed. Most field work, employee training + client home visits occur on weekdays (1-5 days). Non-field work is typically 1-2 days per week (home or Jackson office). No overnight shifts required. EMPLOYMENT TYPE: Long-term employment opportunity for a full-time RN with experience in home health, case management, and administration and/or supervision of nurses and caregivers. Demonstrated experience in -or a strong passion for- business leadership, regional growth, and team development is a must. At least 1 year of experience in case management is required; 1 or more years of experience in home health care, wound care, lab draws, and Oasis assessments is a plus. COMPENSATION: $75,000 - $90,000 annual salary, plus quarterly bonus opportunities. Salary determinations are based upon home health and supervisory experience, case management experience and travel radius / geography coverage, business growth results, certifications, and overall fit. QUALIFICATIONS: Graduate of an accredited nursing program with a current license as a Registered Nurse in the state(s) of practice. Baccalaureate degree in nursing or related field preferred. Minimum of 2 years Nursing experience required, with a combination of acute care and home health care/public health preferred. 1 year Experience in case management required, with home health case management preferred. Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus. 1+ year of experience in Business Management + Team Management (of caregivers, nurses, and care team) is a plus, with health service administration highly desirable. CPR & First Aide Certification required; with CPR & FA Instructor/Trainer certification (or willingness to complete once upon hire) a plus. RNs, LPNs, Home Health Aides, and Personal Care Assistants) while directing, coordinating, supervising, and implementing professional and supportive services to home health clients in a safe, cost-effective manner. RNCM services include starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations and reviews, and in rare coverage situations, may require nursing visits. Communication & Collaboration Facilitate effective communication among patients, families, physicians, and care teams. Serve as a liaison to educate clients and families on available services and community resources. Promote a culture of compassion, safety, and excellence in care delivery.
