About The Position

This hybrid role requires a mix of case management, recertifications, s tarts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. At least 1 year of Case Management experience and home health agency (or equivalent) experience are required . Must have experience with OASIS and NON-OASIS assessments. 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus. To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Nurse Supervisor to join our agency and serve existing and returning clients throughout 4 counties in NorthEast Ohio. This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). This is a Part-time, long-term employment opportunity that can be built to fit your schedule (with potential to expand into permanent Full-time position after 30-90 days). For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported.

Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred.
  • Willingness to travel throughout all stated counties in this service area to care for clients is required.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Minimum of one (1) years of nursing supervisory experience.
  • Home care experience preferred.

  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.

Responsibilities

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
