About The Position

This hybrid role requires a mix of case management, recertifications, s tarts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. At least 1 year of Case Management experience and home health agency (or equivalent) experience are required . Must have experience with OASIS and NON-OASIS assessments. 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus. To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Nurse Supervisor to join our agency and serve existing and returning clients throughout 4 counties in NorthEast Ohio. This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). This is a Part-time, long-term employment opportunity that can be built to fit your schedule (with potential to expand into permanent Full-time position after 30-90 days). For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported. As an RN Case Manager / Nurse Supervisor, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes. With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team. You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred.
  • Minimum of one (1) years of nursing supervisory experience.
  • Willingness to travel throughout all stated counties in this service area to care for clients is required.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.

Nice To Haves

  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Home care experience preferred.

Responsibilities

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
