Requirements

  • Current and valid Registered Nurse (RN) license in the state of Iowa.
  • Current CPR Card.
  • Valid driver’s license, reliable vehicle, and auto insurance.
  • Strong organizational, time management, and problem-solving skills.
  • Excellent communication and interpersonal skills, both written and verbal.
  • Proficiency in electronic health records (EHR) systems.
  • Ability to work independently and as part of a team.

Nice To Haves

  • One year of experience in home health or a related setting (preferred).

Responsibilities

  • Conduct thorough assessments of patient needs and develop personalized care plans.
  • Manage a caseload of home health patients, including skilled nursing, physical therapy, occupational therapy, and speech-language pathology.
  • Obtain and review physician orders, referrals, and medical records.
  • Educate patients and families on disease management, medication administration, and self-care techniques.
  • Monitor patient progress, identify potential complications, and adjust care plans as necessary.
  • Ensure compliance with all regulatory and accreditation standards.
  • Maintain accurate and timely documentation in electronic health records.
  • Collaborate effectively with physicians, nurses, therapists, and other members of the healthcare team.
