Requirements
- Current and valid Registered Nurse (RN) license in the state of Iowa.
- Current CPR Card.
- Valid driver’s license, reliable vehicle, and auto insurance.
- Strong organizational, time management, and problem-solving skills.
- Excellent communication and interpersonal skills, both written and verbal.
- Proficiency in electronic health records (EHR) systems.
- Ability to work independently and as part of a virtual team.
- Reliable high-speed internet access and a dedicated workspace at home.
Nice To Haves
- 1 year of experience in home health or a related setting (preferred).
Responsibilities
- Conduct comprehensive assessments of patient needs and develop individualized plans of care.
- Coordinate and manage a caseload of home health patients, including skilled nursing, physical therapy, occupational therapy, and speech-language pathology.
- Obtain and review physician orders, referrals, and medical records.
- Educate patients and families on disease management, medication administration, and self-care techniques.
- Monitor patient progress, identify potential complications, and adjust care plans as needed.
- Ensure compliance with all regulatory and accreditation standards.
- Maintain accurate and timely documentation in electronic health records.
- Collaborate effectively with physicians, nurses, therapists, and other members of the healthcare team.
