Requirements

Graduation from an accredited School of Nursing; BSN required, MSN preferred.
Active RN license from the North Carolina Board of Nursing.
Minimum 2 years of relevant clinical experience; prior Case Management experience strongly preferred.
Basic Cardiac Life Support (BCLS) required.
Case Management certification highly recommended within 2 years of employment.

Responsibilities

Identify patients who would benefit from Case Management through initial screening and assessment of discharge needs.
Collect and evaluate patient information via interviews, chart reviews, and care‑team discussions to determine required interventions.
Develop, implement, and monitor individualized Plans of Care in collaboration with nursing, physicians, multidisciplinary teams, and community partners.
Lead and coordinate the discharge planning process to ensure smooth, timely, and cost‑effective transitions.
Participate in multidisciplinary rounds, driving patient progression and communicating discharge planning updates.
Document all Case Management activities in the Electronic Medical Record and communicate essential information to providers and payors.
Monitor the medical necessity of admissions and ongoing care; collaborate with Utilization Review when level‑of‑care changes are needed.
Identify system issues, barriers, or trends impacting care; propose solutions and support quality‑improvement initiatives.
Apply age‑appropriate and developmentally appropriate care‑planning and communication techniques.
Guide patients and families through post‑discharge options based on payer guidelines, community resources, and clinical needs.
Build and maintain strong partnerships with physicians to advance the organization’s mission and enhance patient outcomes.
