Requirements

  • Graduation from an accredited School of Nursing; BSN required, MSN preferred.
  • Active RN license from the North Carolina Board of Nursing.
  • Minimum 2 years of relevant clinical experience; prior Case Management experience strongly preferred.
  • Basic Cardiac Life Support (BCLS) required.
  • Case Management certification highly recommended within 2 years of employment.

Responsibilities

  • Identify patients who would benefit from Case Management through initial screening and assessment of discharge needs.
  • Collect and evaluate patient information via interviews, chart reviews, and care‑team discussions to determine required interventions.
  • Develop, implement, and monitor individualized Plans of Care in collaboration with nursing, physicians, multidisciplinary teams, and community partners.
  • Lead and coordinate the discharge planning process to ensure smooth, timely, and cost‑effective transitions.
  • Participate in multidisciplinary rounds, driving patient progression and communicating discharge planning updates.
  • Document all Case Management activities in the Electronic Medical Record and communicate essential information to providers and payors.
  • Monitor the medical necessity of admissions and ongoing care; collaborate with Utilization Review when level‑of‑care changes are needed.
  • Identify system issues, barriers, or trends impacting care; propose solutions and support quality‑improvement initiatives.
  • Apply age‑appropriate and developmentally appropriate care‑planning and communication techniques.
  • Guide patients and families through post‑discharge options based on payer guidelines, community resources, and clinical needs.
  • Build and maintain strong partnerships with physicians to advance the organization’s mission and enhance patient outcomes.
