Position Purpose At Saint Francis Hospital, our RN Case Managers ensure patients receive timely, appropriate, and compassionate care throughout their Emergency Department experience. You will coordinate care, assess patient needs, support safe transitions, and advocate for patient well‑being in a fast-moving clinical environment. What You Will Do As an RN Case Manager in the Emergency Department, you will: Streamline patient flow by coordinating tests, consults, and timely transitions while reducing ED wait times. Perform rapid medical necessity and utilization reviews to ensure correct patient status and avoid inappropriate admissions. Lead discharge and transition planning, arranging follow‑up care, home services, or community support as needed. Support high‑risk patients by connecting them with resources that address medical and psychosocial needs. Educate and advocate for patients and families, ensuring clarity, safety, and a positive care experience.
