About The Position

The RN Case Manager (Care Coordinator) is responsible for facilitating and coordinating the interdisciplinary plan of care. This role focuses on evaluating clinical appropriateness, medical necessity, and level of care while ensuring efficient resource utilization. The Case Manager plays a vital role in promoting positive patient outcomes, supporting patient throughput, identifying barriers to care, and coordinating safe and effective discharge planning.

Requirements

Minimum 3 years of recent (within the last year) Case Manager experience in an acute care setting OR 3 years of experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED
Must have 3 years of total acute care experience, with at least 1 year within the past 5 years
Associate Degree in Nursing (ADN) or Diploma in Nursing required
Active Florida RN license or Compact RN license

Nice To Haves

Bachelor of Science in Nursing (BSN) preferred
Case Management Nursing Specialty Certification
Utilization Review

Responsibilities

Conduct comprehensive medical and psychosocial assessments
Develop, implement, and document individualized case management plans
Lead and participate in interdisciplinary care team meetings
Collaborate across departments to coordinate and evaluate patient care
Review admissions for medical necessity using approved criteria
Conduct concurrent and retrospective reviews for third-party payers
Identify and resolve barriers impacting patient flow
Facilitate timely and appropriate discharge planning
Coordinate post-discharge services to ensure continuity of care
Serve as a liaison between physicians, patients, families, hospital staff, and external agencies
