About The Position

As the CIN Care Navigation RN, you will coordinate and optimize care transitions for ACO and patients transitioning from acute care to post-acute care and ultimately, home . You will track patients transitioning into post-acute care facilities to ensure continued quality care. You will work with post-acute providers to monitor and improve patient outcomes. Requirements

• Associate or bachelor’s degree in nursing.
• Must have knowledge of case management and community resources.
• Must be able to function independently and effectively in a fast-paced environment.
• Written and verbal skills are essential.
• Must be able to establish priorities and communicate effectively.
• At least 2 years of previous case management, care navigation, or post-acute care experience.
• Must have active and unencumbered RN in Ohio and Michigan - or be willing to obtain upon hire.

Nice To Haves

• Master’s degree in nursing or other relevant healthcare field.
• Microsoft Office competency
• 2 years ProMedica
