About The Position

The RN Care Manager (Care Coordinator/ACO) is responsible for working with Coulee Medical Center's medical teams to provide quality, comprehensive care to patients with complex health situations. This position’s duties include individual screening, chronic condition(s) education, medical assessments, charting, care planning, stabilization, observation and maintenance, ongoing coordination of follow-up care, and support for an individual’s self-management. The RN Care Manager empowers patients to self-manage their condition, reduce the occurrence of preventable complications, and overall help patients stay out of the hospital. This position will also serve as the ‘population health nurse’ for CMC's ACO program. Remote/hybrid schedule available, with expectations of being onsite at Coulee Medical Center twice weekly. Requirements

• Current RN license to practice in Washington State
• current BLS certification
• Willing to complete additional certification as requested
