About The Position

Community Care is hiring an RN Care Manager to join their PACE Program team. This full-time opportunity in Racine is for someone who enjoys team collaboration, community case management, and being part of a mission-driven organization dedicated to helping elderly individuals and adults with physical and intellectual disabilities live as independently as possible in the community. Community Care is a local nonprofit with over 40 years of experience, coordinating and delivering supportive services to more than 13,000 Wisconsin residents. They work with members to develop care plans to meet their health and social needs, providing compassionate, personalized care that empowers members to live a healthy and independent lifestyle.

Requirements

  • Associates degree in Nursing Required
  • Wisconsin Licensed Registered Nurse
  • Eligibility for long-term care functional screener certification is required
  • WI Drivers License
  • 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity

Nice To Haves

  • Bachelors Degree preferred
  • 2 years of work in community nursing

Responsibilities

  • Completes discipline-specific initial and ongoing assessments.
  • Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
  • Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports.
  • Follow up on all services to ensure quality/coordination and effectiveness of services.
  • Complete all required documentation and charting and assists in the development and maintenance of member records.
