About The Position

Community Care is hiring a RN Care Manager to join our PACE Program team. This is a full-time opportunity at our Racine location. Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community. We coordinate and deliver a full range of supportive services that help more than 13,000 Wisconsin residents live safely, confidently, and with dignity. We are Wisconsin-based with local offices supporting each of the 23 counties we serve. Requirements

  • Associates degree in Nursing Required
  • Wisconsin Licensed Registered Nurse
  • Eligibility for long-term care functional screener certification is required
  • WI Drivers License
  • Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity

Nice To Haves

  • Bachelors Degree preferred.
  • Preferred 2 years of work in community nursing

Responsibilities

  • Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
  • Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.
  • Complete all required documentation and charting and assists in the development and maintenance of member records.
