About The Position

UW Medicine Harborview Medical Center AACS Population Health has an outstanding opportunity for a Registered Nurse (RN) Care Manager Care Manager & Diabetes Resource Nurse. Approximately 70% of the RNCM’s time will focus on care management and 30% on serving as an expert diabetes and care management resource across clinics for the management of complex patients. This blended role strengthens transitions of care and enhances coordination not only across inpatient and outpatient settings but also between primary and specialty care—ensuring continuity, reducing fragmentation, and improving outcomes for patients with complex needs.

Requirements

Current license to practice as a Registered Nurse in the State of Washington
At least two years of RN ambulatory care /outpatient experience or care management experience
Certification as a Diabetes Care & Education Specialist, must be obtained within 6 months of hire

Responsibilities

Ongoing, proactive care management for high-risk, medically complex patients with primary focus on diabetes
Use the nursing process of assessment, diagnosis, care planning, implementation, and evaluation to provide safe, therapeutic, and coordinated care for primary care patients during in-person and virtual interactions
Support patient care transitions from recent hospital or emergency department discharges, specialty to primary care, and reduces barriers to care
Partner with patients and families to develop individualized care plans, support disease self-management, and improve knowledge of their health conditions.
Regular collaboration with providers, pharmacists, hospital and clinic personnel, and diabetes experts ensures appropriate access to services, efficient use of resources, and high-quality care
