WORK SCHEDULE 100% FTE, 40 hours/weekly Day shift, Monday - Friday DEPARTMENT DESCRIPTION The Ambulatory Care Population Health department provides comprehensive nursing care management for high-risk patients across all Harborview primary care clinics. Our team collaborates across the care continuum to ensure safe and effective transitions between care settings, including inpatient units, the emergency department, and specialty clinics. We serve patients with complex medical conditions, as well as significant social and behavioral health needs, striving to deliver coordinated, patient-centered care. Requirements

  • Current license to practice as a Registered Nurse in the State of Washington
  • At least two years of RN ambulatory care /outpatient experience or care management experience

Responsibilities

  • Ongoing, proactive care management for high-risk, medically complex patients with primary focus on diabetes
  • Use the nursing process of assessment, diagnosis, care planning, implementation, and evaluation to provide safe, therapeutic, and coordinated care for primary care patients during in-person and virtual interactions
  • Support patient care transitions from recent hospital or emergency department discharges, specialty to primary care, and reduces barriers to care
  • Partner with patients and families to develop individualized care plans, support disease self-management, and improve knowledge of their health conditions.
  • Regular collaboration with providers, pharmacists, hospital and clinic personnel, and diabetes experts ensures appropriate access to services, efficient use of resources, and high-quality care
