About The Position

Cheer Home Care is seeking an RN Care Case Coordinator to provide high-quality care to older adults in their homes. This role involves collaborating with an interdisciplinary team to develop and implement individualized care plans, providing constant feedback to clients, client representatives, caregivers, and nurses. The coordinator will work closely with the office team, demonstrating excellent communication and problem-solving skills. The primary responsibility is to serve as the care manager, coordinating care among caregivers, RNs, therapists, and outside representatives.

Requirements

Active California RN License
Strong Computer Skills
Ability to multitask, drive, take action
Strong Organizational & Communication Skills
Pass Live Scan Fingerprint Clearance
Valid driver’s license and proof of insurance
Cell phone with Internet Access
Proof of Negative TB results
Willing to Work One Weekend a Month as Back-Up On Call
