About The Position

The RN Navigator Cardiothoracic Surgery works independently plans, implements, and evaluates nursing care in patient <\/div> care situations using standards of care, policy, and procedure but also in conjunction with the CT Surgery department <\/div> and other members of the CT Surgery Team. Provides direct nursing care to patients, assumes teaching and leadership <\/div> responsibilities for CT Surgery patients participates in shared decision making, referrals, pre and post CT Surgery. <\/div> <\/div> Works directly with the Cardiac Surgeons/ PA’s to provide care to CT Surgery patients. <\/div> Coordinates the referral process and introduction of patients into the CT Surgery Program <\/div>  Employs patient teaching to promote health and safety. <\/div>  Evaluates progress toward outcomes. <\/div>  Practices ethically. <\/div>  Receives incoming referrals from physicians and hospitals and responds quickly to ensure a rapid access into the <\/div> system. <\/div>  Triages referrals according to written criteria to assure those patients’ needs are met on a priority level. <\/div>  Communicates back to referring physicians and hospitals the plans for acceptance into the program in an <\/div> effective manner. Also helps coordinate patient out to tertiary care centers as needed. <\/div>  Educates the patient and family regarding the referral process, steps of evaluation, and keeps patient and family <\/div> informed throughout the entire process. <\/div>  Demonstrates leadership in the professional practice setting. <\/div>  Collaborate with patient, family, the inter -professional team, and others in the conduct of nursing practice. <\/div>  Utilizes appropriate resources to plan and provide nursing services that are safe, effective, and financially <\/div> responsible. <\/div><\/span>

Requirements

  • BSN preferred
  • Minimum of 2 years of experience in cardiac care required
  • Licensed as a Registered Nurse (RN) in the State of Florida
  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association
  • Communication and Problem -Solving skills and ability to maintain flexibility
  • Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows
