The Assistant Manager Home Care is a full-time position with Atlantic Visiting Nurse’s (AVN). We’re accredited by the Joint Commission and licensed by the New Jersey Department of Health. Atlantic Visiting Nurse - Home Health services are available to residents of Morris, Union, Essex, Sussex, Somerset and Warren counties in New Jersey. Hospice At Atlantic Visiting Nurse, we believe that those living with advanced illnesses should spend their final days in comfort and peace. Our compassionate, specialized care is designed to minimize patient suffering and to support families and loved ones in their roles as caregivers. As one of the first hospices in New Jersey and the United States, we have more than 40 years’ experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties. When patients and their primary care physicians agree that curative treatment is no longer appropriate or desired, a referral to Atlantic Hospice may be appropriate. Our hospice team works with referring doctors to develop individualized care plans that manage pain, control symptoms, and ensure dignity and respect in the final days of life. We also have nurse liaisons at Morristown Medical Center, Overlook Medical Center, Newton Medical Center, Chilton Medical Center, and Hackettstown Medical Center, as well as other health care facilities in the area, to answer questions and ease the transition to end-of-life care.

Responsibilities

  • Promotes effective case management:
  • Collaborates with CRC and nurse educator
  • Conducts joint visits for complex patients and performance appraisals
  • Makes patient visits when indicated
  • Oversees scheduling of team
  • Monitors work queues and reports to assure documentation is done accurately and timely
  • Conducts case reviews with field staff to assure optimal case management.
  • Maintains departmental productivity levels for assigned staff:
  • Implements measures to assist staff in achieving productivity goals
  • Provides regular feedback to staff.
  • Promotes a safe environment for patients and staff:
  • Communicates policies and procedures to staff
  • Assists staff with addressing age-related safety concerns.
  • Provides feedback and counseling to staff as appropriate
  • Participates in annual performance review
  • Conducts joint visits with new staff to assure competency.
  • Participates in the agency’s Performance Improvement program:
  • Participates in utilization review activities and other audits
  • Identifies potential PI initiatives.
  • Review and develop action plans to improve team outcomes and reduce rehospitalizations
  • Provides timely response, follow up and service recovery to complaints.
  • Promotes activities to ensure delivery of cost-efficient services:
  • Collaborates with Quality Reviewers when indicated
  • Review and approve supply outliers.
  • Assures that staff meets required competencies:
  • Participates in orientation of new staff
  • Promotes role of preceptor among staff
  • Evaluates education needs of staff.
  • Participates in employee recruitment and retention activities:
  • Interviews prospective employees
  • Promotes employee engagement and retention.
  • Assures staff is in a constant survey readiness state:
  • Assure staff is meeting agency policies and regulatory requirements
  • Assure staff is utilizing appropriate infection prevention techniques
  • Assure documentation is completed timely and is comprehensive.
  • Assists with growing agency business:
  • Cross trains to learn new skills required for role
  • Accepts other assignments as requested to support agency.
