We are currently hiring for an RN Assessor (Per Diem) in any of our regional areas! Rate of Pay: Up to $150/Assessment POSITION SUMMARY: Provides assessment of newly referred patients, assesses health needs and eligibility for home care. Performs patient assessments and re-assessments in the home. Communicates with referrers/discharge planners to review eligibility for agency services and to develop and coordinate the plan of care. Reassesses patient’s needs for services and discusses any changes in the plan of treatment with patient physician/family. Prepares written documentation and summaries of the patient’s progress as needed for continuation of home health services. Communicates changes in the patient condition/home environment, social structure etc. as appropriate. Attends professional and required in-service programs annually Participates in the agency CQI/PI Program, as requested. Other assignments within the RN Practice Act. #VFI123

Requirements

  • NYS Licensed and currently registered as a Registered Professional Nurse.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to speak effectively before customers/patients or employees of the agency.
  • Ability to perform mathematical operations as related to medication calculations, units of American money and weight measurement, volume and distance.
  • Good Organizational writing, communication and interpersonal skills.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several variables in standardized situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
