About The Position

The Admission Care Liaison is a DRIVING position supporting Ohio's Hospice South Region which cover the greater Miami Valley and surrounding areas Schedule hours are day/evening shift 10am-10:30pm with every3rd weekend rotation and 1-2 holidays per year We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed Ohio’s Hospice patients will receive quality nursing care and coordination of hospice services, when the RN/LPN Admissions Care Liaison admits the patient into the hospice program efficiently and provides the patient and family with the appropriate resources needed at the end of life. The RN/LPN Admissions Care Liaison will maintain a conversion rate of 75% and skillful use of the electronic medical record by accurate documentation at the point of care.

Requirements

  • Graduate of an accredited nursing school
  • Associate of Science in Nursing (ASN)
  • Bachelor of Science in Nursing (BSN) preferred
  • Active Registered Nurse license in the State of Ohio without any board actions
  • Two years acute care nursing experience preferred
  • CPR Certified
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
  • Computer skills sufficient to properly document services and care
  • Ability to drive during daytime, nighttime or inclement weather
  • Valid Driver's license with Safe Driving Record
  • State minimum automobile insurance coverage
  • Must be able to mass a background check
  • Must be able to pass a 10 panel drug screen

Nice To Haves

  • Bachelor of Science in Nursing (BSN)
  • Two years acute care nursing experience
  • Certification in Hospice & Palliative Medicine (CHPN)
