About The Position

Risk Support Specialist At Rever Grand, our mission is to provide high quality, person-centered services to individuals with intellectual and/or developmental disabilities (I/DD), assisting them to lead dignified, independent lives in the comfort and safety of their own homes and the community in which they live. Rever Grand is seeking a Risk Support Specialist to be a part of our dedicated administrative team that serves the I/DD community. The Risk Support Specialist collaborates with individuals and/or their representatives to create person-centered plans for Direct Support Professionals (DSPs) to use as guides in supporting individuals with their identified risks and medication support needs. This position collects and interprets information for the development of Risk Support Plans and Medication Support Protocols,ensuring completion and submission prior to mandated deadlines.   • High school diploma or equivalent.
  • Three or more years of experience in customer service and/or data entry and analysis, or other related areas.

  • Experience interacting with individuals with intellectual or developmental disabilities a plus.
  • Understanding of health, medical, safety and behavior risks a plus.

Responsibilities

  • Collect and interpret the information provided by the individual and/or their representative for the development of the Risk Support Plan, asking clarifying questions and identifying when further information is needed to gain a thorough understanding of how health, medical, safety and behavior risks present and how to best support the individual with them.
  • Collect and interpret the information provided by the individual and/or their representative for the development of the Medication Support Protocol, asking clarifying questions and identifying when further information is needed to gain a thorough understanding of how DSPs can support an individual with their medication management needs.
  • Review the Individual Support Plan (ISP) and other support documents to gather information that is needed for the development of the Risk Support Plan and/or Medication Support Protocol.
  • Communicate directly with the individual and/or their representative to determine how they prefer to be supported, incorporating those preferences into the plans.
  • Conduct research when needed to ensure that Risk Support Plans and Medication Support Protocols contain thorough information for DSPs to best support individuals with their needs.
  • Develop and write thorough and detailed plans with the information that has been compiled, while ensuring compliance with State guidelines.
  • Conduct annual and as-needed reviews of the individual’s Risk Support Plan and/or Medication Support Protocol, updating these plans with new information when applicable.
  • Meet deadlines for Risk Support Plan and/or Medication Support Protocol completion and submission.
  • Engage in complex conversations when needed to gain and interpret the necessary information for these plans.
  • Communicate via phone, email and/or text message with employees, individuals, representatives and external contacts, demonstrating excellent service skills, knowledge, and professionalism.
  • Utilize data entry systems to respond to and document phone calls, emails, and text messages with strong attention to detail.
  • Maintain strict confidentiality of sensitive and private information.
  • Adjust processes and procedures as needed to comply with changing guidelines.
  • Perform other duties as assigned.
