About The Position

• Monitor and complete monthly reviews, summaries, risk assessments, daily notes, supervisor's notes, SITs, and vocational assessments, etc. • Attend meetings: COS, ISP, SIS, etc. • Maintain up-to-date staffing plans. • Assist Individuals as needed with mealtimes, break times, hygiene, and behavior management. • Ensure that DSPs have completed SIT, mealtime, behavioral, etc., training and are aware of each person's supervision needs and that there is documentation of such. • Complete surveys of daily notes for quality and timeliness. • Maintain close communication with the Coordinator, families, ISCs, Support managers, and other OGC Departments. • Fill in roles as requested by the Coordinator. • Write and monitor implementation of staffing instructions for outcomes/action steps. • Be aware of yellow and white incident reports, seizure reports, and behavior report circumstances. Ensure reports are completed and assist as needed to ensure completeness and accuracy. • Ensure Person Centered language and practice and advocate to maximize individual community inclusion and satisfaction. • Collaborate with other entities and the Supervisor to provide curriculum and outcomes/actions that promote a greater level of independence and community inclusion for the individual. • Provide comprehensive case management to all individuals who live at home with family. • Visit individuals at their job sites, etc., to ensure holistic programming and documentation. • Serve as a mealtime card and Behavior Support Plan trainer. • Responsible for keeping site access code confidential. · Responsible for ensuring that all items that need to be locked up, such as electronics, documents containing information covered by HIPAA, etc. are secured when not in use. · Acquire and keep current OGC medication administration status. · Acquire and maintain OGC driving privileges and be willing and able to transport individuals in your personal vehicle to meetings, etc. · Relay individuals' scheduling information to Supervisor as it becomes available to you. · Complete billing calendars and confirm that daily notes justify the billing. · Be an advocate for individuals. · Keep an open mind and lead staff in supporting individuals with ongoing growth and opportunities in the community and prevent “nesting” at the community site. · In the absence of the hub Supervisor, monitor and ensure the safety of the following: direct supervision of DSPs, meal times, hygiene needs, behavioral management, bus duty, completion of reports, computer access system, clock in/out system, cleaning protocol, emergency procedures, liaison with church personnel, maintaining security of the site and of OGC items, maintaining 1:3 ratio, ensuring personal cell phones are not used and relay need for supplies to the Coordinator.
