About The Position

This position is based in the Infectious Disease Group Practice at the University of Colorado Hospital. The Retention Coordinator provides retention in care and access adherence monitoring services to patients living with HIV.

Requirements

  • Bachelor’s degree in any field from an accredited institution.
  • A combination of education and related technical/military/paraprofessional experience may be substituted for a bachelor’s degree on a year for year basis.
  • Applicants must meet minimum qualifications at the time of hire.
  • Ability to communicate effectively, both in writing and orally.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.
  • Outstanding customer service skills.
  • Strong organizational and multitasking skills.
  • Ability to work independently as well as collaboratively within a multidisciplinary team with diverse populations.

Nice To Haves

  • Bachelor’s degree in public health, social services, or another related field.
  • Professional experience providing services to people living HIV.
  • One (1) year of case management or related experience.

Responsibilities

  • Assess patient needs for retention services by reviewing clinical information, conducting outreach, and identifying social, behavioral, and systemic barriers to care.
  • Provide retention and re-engagement services, including outreach, follow-up, appointment coordination, and development of individualized retention plans to support continuity of care.
  • Maintain regular communication with patients to promote adherence to care and treatment plans and track patient progress and outcomes.
  • Document all assessments, services, and outcomes accurately in the electronic medical record and relevant tracking systems.
  • Support care coordination by assisting patients with referrals to internal and external services, including mental health, substance use, housing, and community resources.
  • Collaborate with clinical teams, support staff, health departments, and community partners to ensure continuity of care and facilitate patient re-entry into services.
  • Participate in quality improvement, research, and program development activities, including data collection, monitoring program effectiveness, and identifying opportunities for service improvement.
  • Ensure all activities comply with organizational policies, confidentiality requirements, and regulatory standards.
