This role works closely with therapy, nursing, and interdisciplinary teams, and requires strong knowledge and experience with the Minimum Data Set (MDS) to ensure accurate documentation and compliance with regulatory standards.

Requirements

Graduate of an accredited school of nursing
Licensed by the Kentucky Board of Nursing as a registered nurse or Licensed Practical Nurse
Current CPR certification
Two to four years of related experience and/or training, or equivalent combination of education and experience

Responsibilities

Conduct comprehensive assessments to determine residents’ functional abilities and rehabilitation potential.
Develop individualized restorative nursing care plans based on clinical findings, goals, and regulatory requirements.
Supervise CNAs and unit staff to ensure safe, efficient care delivery
Complete timely and accurate documentation reflecting resident progress, participation, and outcomes.
Ensure restorative programs align with federal and state regulations for long-term care.
Maintain records to support MDS accuracy and reimbursement compliance.
