About The Position

Provide supervisory direction to staff assigned to Park Lawn Center. Ensure Compliance with all Park Lawn, IDPH, DHS, and funding and review agencies’ requirements. Direct Staff assignments, schedule staff, follow up to ensure staff compliance. Maintain safe, clean, and comfortable living environment. Develop functioning team to promote well-being of assigned residents. Position is responsible for shift employee staffing and resident care at Park Lawn Center.

Requirements

  • High School Diploma
  • Three years’ supervisory experience preferably in IDD field
  • No disqualifying events on criminal background check
  • No disqualifying events on Health Care Workers Registry
  • Pass pre-employment physical, drug and alcohol test
  • Valid Illinois Drivers’ license

Responsibilities

  • Schedule and supervise assigned staff
  • Maintain functioning team to implement residents’ goals
  • Personally cover shifts and days when staff are not available
  • Understand written and oral English Language
  • Record observations in participants’ files
  • Write legible, clear, and accurate observations in standard English
  • Lift 50 pounds without assistance
  • Stand, sit, walk, reach, grip, bend and lift as assessed by job analysis screening
  • Remain free of alcohol and/or illegal substance use while working; Become CPR/1st Aid certified and remain certified throughout employment
  • Direct assigned staff in completion of all tasks necessary to maintain compliance with Park Lawn procedures and policies; Illinois Department of Public Health (IDPH) rules and regulations; Funding and Certification organization standards.
  • Schedule work hours (inclusive of approval of time off) of assigned staff to maintain residents’ safety, promote residents’ goals, and ensure Park Lawn compliance with DHS and IDPH requirements.
  • Ensure development and maintenance of assigned staff skills through completion of On the Job Training (OJT) requirements, attendance at training events, staff meetings, and in service events.
  • Complete employee performance evaluations; develop employee plans of correction; and initiate corrective action and/or termination recommendations with input from Manager and Human Resources.
  • Interview perspective staff and make hiring recommendations to Manager.
  • Maintain professional and cordial relationships with all stake holders including resident’s family and guardian; other service providers; and other Park Lawn personnel.
  • Maintain work schedule to ensure on-going contact with staff on all shifts, including weekends to promote resident services and activities as well as compliance with Park Lawn policies and procedures.
  • To coordinate and implement written procedure(s) in the event of an emergency (i.e., tornado, fire, and emergency evacuation of individuals).
  • To communicate operational and programmatic problems to the Residential Staff Manager and/or QIDP, and/ or administration.
  • To ensure that the Residential Staff Manager and other residential supervisors are notified via documentation and/or phone call when there are: Call Offs (sick, etc.) Emergency Hospitalizations No Calls, No Show Emergency Exits (fire alarm sounds) Under Ratio Building Problems (floods, leaks)
  • To make appropriate staff coverage in the event of call offs, no shows, etc.
  • To ensure that programs, activities and appointments are going according to schedule and document reasons when a schedule is changed or altered.
  • To ensure that regulatory guidelines and agency policies and procedures are being followed throughout the facility.
  • To ensure that open communication is maintained with nursing concerning medical conditions of the residents.
  • To ensure that parents with concerns are referred to the QIDP in charge of the resident’s case, or if unavailable the Residential Staff Manager or Administration, and document concerns in the shift log.
  • Monitor the entire facility to ensure programs are being implemented, that the facility is functioning correctly and active treatment is maintained.
  • Ensure that data is being collected and all staff process forms in the facility. This includes, but is not limited to the following: Programmatic Data Daily Progress Notes Medical Data Behavioral Date Dietary Data Incident Reports Shift Log
  • Ensure that all behavior management, eating, diets, toileting, and therapeutic programming are consistent and ongoing.
  • Assist to train employees with QIDPs, Residential Staff Manager, Therapists, and Consults.
  • Attend training sessions, which includes but is not limited to the following: All mandatory agency training Shift meeting/shift lead meeting Residential training offered by the staff trainer Outside training offered by Park Lawn Training required by regulatory agencies Monthly in-service training
  • At times, will be required to remain and work until adequate staff ratio is present.
  • To provide other needed services as deemed necessary by the Residential Staff Manager and Administration.
  • Responsible for administrative oversight in absence of Residential Staff Manager and/or administration.
  • Maintain accurate and timely documentation as required; including staff attendance, training, employee counseling, and staff meetings.
